Suzanne Walker, MD

Suzanne Walker, MD, serves as the Medical Director at the BlackRock Center for Health and Well-being in New York, New York. A graduate of Albany Medical College, Dr. Walker is a board-certified Family Medicine physician.

Dr. Walker completed her internship and residency at New York Presbyterian Columbia University Medical Center, during which she was appointed Chief Resident of the Family Medicine program, showcasing her exceptional leadership.

For more than a decade, Dr. Walker provided comprehensive care to multiple generations in underserved communities throughout New York City. This experience fueled her passion for practicing preventive health care to support her patients in managing their chronic conditions. Thereafter, Dr. Walker continued to care for patients in a private practice setting for eight years, championing a high-quality, patient-centered, accessible, and team-based approach to medical care.

Since 2018, Dr. Walker has been the dedicated Medical Director at the BlackRock Center for Health and Well-being. Her approach to medicine is characterized by a commitment to preventative care and collaborative decision-making, treating patients as whole individuals.

In her free moments, Dr. Walker enjoys gardening and spending quality time with family and friends.

Amy Polansky, PT, DPT

Michael Felcone, PA

Michael Felcone is a physician assistant at the Center for Health and Well-Being. Upon completing his medical training at Hahnemann University in Philadelphia, Michael moved to New York to practice primary care for adults. As the former Chief Physician Assistant of Mount Sinai’s Center for Post-COVID Care (the first of its kind in the nation) coupled with time spent working alongside Mount Sinai’s Chief of Ambulatory Care and Chief of Geriatric Medicine at Mount Sinai Beth Israel, Michael has a demonstrated experience in the fields of ambulatory, endocrinology, geriatric, and sleep medicine.

Prior to practicing medicine, Michael was a professor of music at the Manhattan School of Music in New York. He received his musical training in both theory and piano performance at the Peabody Conservatory of Johns Hopkins University in Baltimore, MD. Passionate about the intersections between medical and musical arts, Michael fervently believes the arts are the most underused form of medical therapy.

In his free time, Michael—an inveterate New Yorker—takes advantage of the city’s cultural events, especially the museums, theater, and musical offerings.

Hanna Chapman, NP

Hanna is a board-certified Family Nurse Practitioner. She earned both her Bachelor and Master of Science in Nursing degrees from Vanderbilt University, where she was selected as an Ingram Scholar. Hanna was also awarded a HRSA Scholarship and has since practiced internal medicine for the past eleven years. Her clinical expertise includes preventive care, chronic disease management, cardiometabolic health, women’s health, and healthy aging. She is certified in ACLS and BLS and is committed to delivering high-quality, evidence-based care that supports long-term wellness.

In addition to her clinical practice, Hanna has led care delivery innovation at Mount Sinai Solutions and Vesta Healthcare, where she designed and implemented programs that improved access, enhanced quality, and achieved better health outcomes for diverse patient populations.

Known for her thoughtful and collaborative approach, Hanna partners closely with patients to create personalized care plans that promote longevity and empower individuals to take charge of their health.

Amy Polansky, DPT, is a Physical Therapist at the BlackRock Center for Health and Well-Being. With more than 25 years of experience, Amy’s focus areas include vestibular rehabilitation, adult rehabilitation, general and post-operative orthopedic rehabilitation, and treating balance disorders and spinal conditions. Passionate about providing her patients with individualized attention and focused treatment to support optimal musculoskeletal health, Amy continues to advance her education by taking courses related to the latest treatment techniques for rehabilitation of knee, shoulder, and foot disorders, as well as movement impairments and soft tissue mobilization techniques. After earning her Bachelor of Science with honors, in Management from SUNY Binghamton, Amy completed her second Bachelor of Science in Allied Health, as well as her master’s degree in Professional Studies in Physical Therapy from SUNY Health Science Center at Syracuse.

Swaroopa Dundon, LMHC

Swaroopa Dundon, LMHC, is a licensed mental health counselor with many years of experience working with individuals, couples, and families. Swaroopa has experience working collaboratively as part of a multi-disciplinary team of psychiatrists, social workers, and nurse practitioners treating an array of diagnoses including depression, anxiety, trauma, substance use, and eating disorders.

Swaroopa brings a multicultural, multilingual, and integrative perspective to her therapeutic style—she has lived and worked in different parts of the world, including Hong Kong and India, and speaks several South Asian languages. Swaroopa has a client-centered approach to her work and utilizes techniques from CBT, DBT, and family systems to inform treatment.

In addition, Swaroopa is a certified yoga teacher and has extensive training and certification in couples and family work.
